Welcome to our website dedicated to the Access and Navigation Project started by the Eastern York Region North Durham (EYRND) Ontario Health Team (OHT) in 2021. The intended outcome of the project is to implement quality improvements to the current navigation processes with the goal that a caller can have a standardized experience no matter which organization (211, Access York, ConnexOntario, Home and Community Care Support Services Central and Central East, Your Support Services Network, Central Healthline) is called. Specifically, callers are offered a warm transfer to a number of OHT providers who have agreed to be involved in phase 1 of the project, and when a warm transfer is not possible or not the caller’s preference, a standardized referral form is used to enable the caller to receive a call back from a provider agency. With these changes, the goal is that callers can be linked, with as few steps as possible, to appropriate services that are able to meet their need(s).

In 2023, the project became a collaboration between EYRND OHT, Northern York South Simcoe (NYSS) OHT, and Western York Region (WYR) OHT. Now, anyone who lives in York Region, West Gwillimbury, and Uxbridge can benefit from an improved process for finding health and social services. These three Ontario Health Teams continue to establish agreements with additional service providers so that more services can be available through warm transfer and standardized referral forms.
